The new, Midwest Laboratories, Inc. SUBMITTAL FORM  will assist Midwest Laboratories Representatives to schedule your sample analysis in advance of its arrival at the laboratory.  In addition, it gives you, the customer a way to keep all of your paperwork saved to your computer.   

Submitting Paperwork   

 

1) Click on the link below and fill out the Microsoft Excel Form and  save the Form to your computer.

2) After filling out the Template - attach it to an email to Brent Pohlman - Midwest Laboratories Inc. brent@midwestlabs.com  

 

Place Copy of Submittal Form with your samples 

  

3) Make sure to print a copy of the form and place it with your samples.
